The Incident in Vista De Oro will be handled by the Districts Insurance company, SDRMA, and they will make the determinations. Please fill out the attached claim form and turn it in to the District Office as soon as possible. The District office is at 2122 9th Street, Suite 110, and also has printed Claim forms available to fill out. Office hours are 10am-3pm, Monday through Friday. If you need to turn it in at a different time, please call the front desk at 805-528-9370, or contact Laura Durban at 805-528-9435 or ldurban@losososcsd.org, and we will do our best to assist with getting claim forms turned in.
If there is a person who owns the property and a tenant, then each should fill out a separate claim form but that’s the only time more than one claim form should be submitted per property.
For each claim filed per property, the claim form needs to include all of the damages by each person in the property/household.
